The real basis on which to assess risk should always be the individual driver. It&#8217;s fair to look at the person&#8217;s experience, driving record, how far he or she drives every year, etc. That way you reward the good drivers with lower premiums and hit the bad drivers with higher premiums. This ends the discriminations of higher premiums for people living in predominantly black or Latino communities.</p>
<p>It would be great if we could see this change sweeping across the US, not just in auto insurance, but for all classes of insurance. Unfortunately, the insurance industry has fought the change tooth and nail wherever it has been proposed. Lobbyists with deep pockets have been able to keep the legislators at bay. <p>There are obvious explanations. The hospitals and clinics in different areas attract doctors and healthcare providers with different levels of experience and expertise. Operating costs will also change with local conditions. The level of support for public facilities and programs from local government naturally varies depending on the local tax take and political factors. These affect the rates for services the insurers can negotiate with the local provider networks. And then there are all the intangible factors based on the wealth or poverty of an area, the percentage of people without current <a href="http://www.healthinsurancebible.com/zip-codes-and-premiums.html">health insurance</a>, and so on. Put everything together and profiling by geography may produce very different results. This leaves us with an uncomfortable reality. As it stands, the <a href="http://www.healthinsurancebible.com/">health insurance</a> industry is unregulated. It can charge what it likes using whatever factors it wishes to consider significant. As and when the healthcare reforms pass through Congress, some practices that produce unfairness will disappear, e.g. no more discrimination based on gender, no more discrimination by denying coverage to people with pre-existing conditions, no more caps on lifetime benefits, and so on. This is the Genetic Information Nondiscrimination Act (GINA) and this November sees it finally come into force. Because it affects both employers and the insurance industry, this has been a hard-fought change and was only signed into law last year. Now it should prevent you from obvious discrimination. Sadly, it does not rule out discrimination by backdoor means. If an employer overhears you talking at the water cooler or routinely surveys local news including the obituaries, it is legal to use this information. But, overall, you should find some improvement. It covers two different situations with the same type of result. Firstly, it prohibits employers from asking you to go through a genetic test or making genetic information the basis of deciding whether to hire, promote or fire you. Secondly, it prohibits insurers from testing or demanding genetic information about you or your family in deciding whether to offer you coverage, in setting the premium rate and level of the deductible, or continue the cover.<br />

Let&#8217;s be completely clear. The law does not care who is asking for the information If it is going to be used for either purpose, the asking is unlawful. Whether you are an employee or self-employed, it is necessary that you have a good health insurance coverage to cover your medical bills. However, there is no unique health insurance plan good for every one; benefits and costs vary from an individual to another (due to age, medical condition, etc.). To make <span id="more-55"></span>a good choice, you need to know what benefits you are looking for, and examine each plan to find the one that best responses to your needs.</p>
<p>Although you have many options in choosing your health insurance, finding the right plan can be difficult. In general, individual health insurance is a form of contract between you and an insurer (insurance company )to repay all or almost all of your medical bills, which may includes hospitalization, medications, dental care, seeing a specialist, and certain therapies (radiotherapy, chemotherapy, etc.).  Whatever your needs, you will most likely have to choose one of these plans, Fee-for-service, HMOs (Health Maintenance Organizations), or (PPOs) participating provider organization.</p>
<p>Fee-for-service &#8211; also known as indemnity plans, is a type of insurance plan where you, patient, have to pay all medical expenses out of your own pockets, and then request a reimbursement from your insurance company. These types of plans have their advantages and disadvantages.</p>
<p>Advantages: they offer more flexibility in choosing your own doctor. You can decide the time to see your health care provider, and what type of treatment you want; as long as you remain in the limit that your insurer will repay</p>
<p>Disadvantages: in indemnity plans, most doctors require upfront payment, so you have to submit claim forms to the insurance company to receive a reimbursement. That requires paper work, and sometimes many phone calls. Fee-for-service plans offer limited benefits; they do not cover annual physical exam and educational programs.</p>
<p>HMOs (Health Maintenance Organizations) &#8211; Health maintenance organizations (HMOs) are managed care plans that offer health care coverage to their members through hospitals, doctors, and other health care providers that are in their network. That is, having their service, you are limited to members of their network.</p>
<p>Advantages: unlike Fee-for-service plans, you do not have to pay up front; although some of them require a copayment. You do not need to submit forms after forms to receive reimbursement. In addition, HMOs usually charge a lower cost.</p>
<p>Disadvantages: you can use only health care providers who are associated with the organization. Most HMOs (Health Maintenance Organizations) tend to disapprove certain treatments. Although some HMOs accept their members to see physician or specialists who are not in their network, they often charge you additional costs.</p>
<p>(PPOs) participating provider organization &#8211; also known as Preferred Provider Organizations,  is a form of managed care organization of physicians , hospitals, clinics and other health care providers that sign a contract with an insurer to provide health services to its member at reduced rates .  Usually, PPOs cost more than traditional HMOs, but offer more options to their members.</p>
<p>Advantages: Preferred Provider Organizations provide more flexibility to their members; they have a bigger network of doctors and hospitals. You can take service from health care providers that are not part of their networks (certain charges often apply). You pay Lower copayments for care from primary care physicians. In addition, you do not need a referral to see a specialist.</p>
<p>Disadvantages: PPOs cost more than traditional HMOs. You will more likely to make co-payments (usually from $10 to $30) when you visit a health specialist.</p>

After reading this message you will never going to have words; expensive and health insurance in the same sentence.</p>
<p>          As you already know health insurance costs are at <span id="more-52"></span>highest they have ever been and there is no sign of them slowing down. More and more Americans are forced to cancel their coverage simply just because they cannot afford it.</p>
<p>Who are the uninsured?</p>
<p>•	Approximately 46 million Americans, or 15.7 percent of the population, were without health insurance in 2004 (the latest government data available). </p>
<p>•	The number of uninsured rose 800,000 between 2003 and 2004 and has increased by 6 million since 2000. </p>
<p>•	The increase in the number of uninsured in 2004 was focused among working age adults. The percentage of working adults (18 to 64) who had no health coverage climbed from 18.6 percent in 2003 to 19.0 percent in 2004. An increase of over 750,000 in 2004. </p>
<p>•	Nearly 82 million people &#8211; about one-third of the population below the age of 65 spent a portion of either 2002 or 2003 without health coverage. </p>
<p>•	The number of uninsured children in 2004 was 8.3 million &#8211; or 11.2 percent of all children in the U.S. (1). </p>
<p>You might say that I have great coverage that I am happy with… that’s totally fine. </p>
<p>            For past sever years average rate increase for health insurance was 16.2% and what if it keeps on going? If you are right now paying $500 per month for your health insurance in three years from now you would expect to pay over $780 for the same plan.  Wait… we all know that insurance companies consistently decrease their benefits and increase co-pays and deductible. Therefore you will pay more for less coverage. By the way if you keep same plan for over five years you will pay over $1000 a month just for your medical coverage. What if you use your Health Insurance?&#8230; Chances are if it is not for a regular doctor visits or a check ups it would be considered pre-existing condition. That means your chances of changing to a more affordable coverage in the future will be nearly impossible. That is one of the main reasons people cancel their health insurance because they were diagnosed with something or taking a prescription medication and the insurance company kept raising their rate until they could not qualify for any other coverage and could not afford the one they had. </p>
<p>             Now you are saying I do not need coverage my spouse works for a company and I have group coverage… Great.</p>
<p>What would happen if your spouse left that job or the company stopped providing benefits? Probably the most obvious things that you can see how much that group coverage is really costing you. Next time check how much is deducted out of the paycheck for health coverage, especially for dependents. Group plans do cost more money because by law they are what are called “guaranteed issue”. That means you can have serious medical conditions and still get coverage. Insurance companies have to follow the law and they know they have to accept everyone who works for a large company, therefore they do charge more money for coverage. The biggest problem is not the cost of group health insurance it is what happens if some one, while on the group plan, is diagnosed with a condition or starts to take prescriptions medications. We get back to same issues as mentioned before, unable to qualify for health insurance in the future. There are people that want to leave their job but they cannot because they are going through treatment and cannot to pay for it on their own.</p>
<p>              There is another solution… Some might save, so what is the point of even having health insurance. Once you diagnosed with something and insurance company is going to keep raising rates to the point where I am going to have to cancel it anyway. Especially if something does happen and I have to use my coverage I might not be working and I might not have income. Is my insurance company is still going to keep raising my rates? YES.</p>
<p>Before you think about cancelling your coverage consider this. Here are some statistics</p>
<p>•	A recent study by Harvard University researchers found that the average out-of-pocket medical debt for those who filed for bankruptcy was $12,000. In addition, the study found that 50 percent of all bankruptcy filings were partly the result of medical expenses. Every 30 seconds in the United States someone files for bankruptcy in the aftermath of a serious health problem. </p>
<p>•	Illness and medical bills caused half of the 1,458,000 personal bankruptcies in 2001, according to a study published by the journal Health Affairs.</p>
<p>•	Average day in the hospital is $7500 per day.</p>
<p>            How can you save up to 47% on your health insurance? Simple… You probably already heard of Health Saving Accounts. They are becoming more and more popular everyday. With the way health insurance prices are moving today Health Saving Accounts are the only way to keep your coverage, save hundreds per month on your health insurance and still have a peace of mind. </p>
<p>             To this day I was not able to hear a good definition that everyone can understand. I will do everything I can to make it simple to understand. The easiest way to understand Health Saving Accounts is to think of them as Roth IRA or your Company’s 401k plan. Instead of giving your money away to insurance company you get to keep it more of it for yourself. The way HSA plans work is there health insurance combined with savings account which works in a similar way to your retirement account. There tremendous benefits to have HSA qualified health plan. First all the money that you put in to your HSA account is 100% tax deductible and it is your money that rolls over year after year. At the age of 65 and up if you have not used up all of your HSA money you can roll it over in to your retirement account. Second your health insurance costs are going to be cut almost in half. For example if you had Health Insurance plan with $2500 deductible now and it is costing you $300 per month the same plans with HSA qualified plan, now will cost you only about $160 per month. The reason you save so much money with HSA qualified health plan is because HSA qualified plans do not cover anything until the deductible is met. There are exceptions depending on the Health Insurance Company.  Some insurance companies will pay for your once a year physical before you meet your deductible.</p>
<p>             Let take an example of how HSA qualified plan could benefit you. Let take some actual numbers from actual health insurance company. In this example I am going to use HSA plans from company called Assurant Health. Assurant Health is leader in Health Saving Accounts and they one of the first companies to implement them. The main reason is that Assurant Health is part of the world’s largest financial company that sets up retirement accounts. In this example I am going to use a family of four, husband 46, wife 42, kids are 12 and 16. On a regular family plan with $2500 deductible, maximum out of pocket of $5500, co-insurance of 80% and doctor visits covered with $35 co-pay, they are going to pay $676.40. Something to keep in mind that all of the regular PPO plans that are available on the market today have family deductible which is double of individual deductible. That means that if you have a plan with $2500 deductible and $5500 maximum out of pocket that means that your family deductible is $5000 and your family maximum out of pocket is $11,000. When we are comparing HSA qualified health plans there is only one deductible, once you meet it you are covered at 100% on the most plans. There are some companies and plans that you still might be responsible for the percent age of the bill until you reach your maximum out of pocket. Most HSA plans do not have maximum out of pocket that meant once you met your deductible you are covered at 100%, it’s that simple. The same plan with $5700 deductible for the entire family with HSA qualified health plans will only be $491.64 per month. For the total monthly savings of 184.76 per month. Also your maximum out of pocket will decrease from $11,000 on a regular plan to $5700 with HSA health plan. That’s yearly savings of $2,217.12 and additional savings of $5300 on the maximum out of pocket. (that’s if you have had to use the plan for emergencies) The main reason for starting HSA health insurance is for Saving Account and being able to put money in to account, at your discretion, tax free. You can put money in to HSA qualified account up to your deductible and you do not have to put any money in to that account if you do not want to. <p>Here is audio of President Obama on a conference call where a blogger from Maine asked Obama a specific question about HR 3200 &#8211; the Democrats&#8217; Health Care bill &#8211; the bill he is pushing for passage. The caller said he kept running into an Investors Business Daily article that claimed Section 102 of the House health legislation would outlaw private insurance. The caller asked: &#8220;Is this true? <p> Barack Obama&#8217;s ambitious health care plan is fairly simple and straightforward. His plan seeks to dramatically and swiftly increase the number of people that have health insurance. He insists that this plan will save the typical American family approximately $2500 in annual costs. Since the average Ohio health insurance premium is less than most other states, savings to Ohio residents may average less than $2500.</p>
<p>The Obama pla<span id="more-49"></span>n is designed to give the federal government more control over health care decisions and dollars, a major difference from the current decentralized system of employer-based insurance and state-based insurance regulation. Here in Ohio, health insurers have been effectively held in check by the Ohio Department of Insurance. This, however, is not the case in many other states.</p>
<p>The Obama Plan</p>
<p>Many parts of the Obama plan resemble initiatives from the Clinton health plan of 1994 and the Kerry Health plan of 2004. <br />Essentially, Obama&#8217;s health care plan is divided into three sections:</p>
<p>1. Modernizing the US health care system to lower costs and improve quality <br />2. Promoting prevention and strengthening public health <br />3. Quality, portable and affordable health coverage for every person</p>
<p>The &#8220;Savings&#8221;</p>
<p>The $2500 in savings will come from health care reform, using some of the following initiatives:</p>
<p>*Making health insurance universal, which may reduce spending on uncompensated care. <br />*Improving management and prevention of chronic conditions. <br />*Increasing insurance industry competition and reducing underwriting costs and profits. <br />*Providing reinsurance for catastrophic coverage, which will reduce insurance premiums.</p>
<p>Shifting Cost Burden</p>
<p>While all of these ideas are feasible, the underlying theme seems to be simply shifting some of the cost burden from the private sector to the government. And of course, much more control of our health dollars and decisions would come from Washington D.C and not Anthem or UnitedHealthCare.</p>
<p>The Obama plan will actually compete directly with Ohio private health insurance companies in a &#8220;National Health Insurance Exchange.&#8221; The federal government (not health insurance carriers) would determine the quality of health benefits that Americans would receive. And these new rules would apply to both the new national health plan and all participating private health plans.</p>
<p>Preventative Coverage Would Be Emphasized</p>
<p>Obama&#8217;s health care plan will encourage &#8220;healthy lifestyles&#8221; with specific emphasis on wellness. Employer wellness programs will be increased, and cafeterias and vending machines in the workplace may see healthier food.</p>
<p>School-based health screening programs may increase along with increased support for physical education.</p>
<p>For Ohio individuals and families, the Obama plan would require preventative services on many federally-supported health programs such as Medicare, Medicaid and SCHIP. One benefit may be possible discounts to on health insurance premiums for enrollment in wellness and prevention programs.</p>
<p>Currently, some Ohio individual health insurance policies offer a similar discount, such as Anthem&#8217;s Lumenos Health Incentive Account (HIA).</p>
<p>Ohio Group Health Insurance</p>
<p>Employer-based health insurance would radically change under the Obama plan. Here in Ohio, both small and large employers are able to choose among many different health plans for their employees. The Obama plan would force employers to offer a specific level of health benefits to their employees or pay a tax to finance a national health program. Currently, the amount of provided health benefits and the size of the tax have not been specifically discussed.</p>
<p>Perhaps the best and most economical health insurance plan for Ohio residents would be a concept already in place&#8230;HSAs (Health Savings Accounts). Thus, instead of imposing a top-down change on the health care system, it would seem to be prudent to transfer direct control of health care dollars to individuals and families. This would allow Americans to choose their own health plans and benefits, while making health insurance companies compete directly for consumer&#8217;s dollars by providing a real value to patients.</p>
<p>All of this could be accomplished by specific tax and regulatory changes designed to utilize the power of free-market competition. Health care spending could be reduced, preventative treatment could be emphasized and portability could be promoted. Reforming the tax treatment of health insurance and aiding employers that help their employees buy health insurance would help quite a bit.</p>
<p>For now, Ohio health insurance rates are remarkably low compared to many other states. There are many reputable health insurance companies that offer a wide array of policies, including Health Savings Accounts. That shouldn&#8217;t change much for the next two years. In 2011, things might change&#8230;hopefully, for the better.</p>

1) Is health insurance compulsory for everyone?<br />
2) What happens if someone cannot afford it?<br />
3) In the event that a medical procedure needs to be done, does health insurance cover all the bills? Does the patient need to pay anything extra?<br />
4) Does the patient have any say over what kind of procedure he can take? Say if 2 treatments are available for his condition, can the patient choose the more expensive treatment? And if so, is it covered by the insurance? </p>

<p>Does a health insurance contract state that it will only cover the &quot;normal&quot; rates for a procedure? For eg. if there are 2 possible treatments for a disease, 1 of which is more expensive but more effective than the other, will the patient only be covered by the LESS expensive one? </p>
<p>Or is it a case in which the patient can opt for the more expensive one and &quot;top-up&quot; the difference? </p>
